Add-in not starting

Running Outlook 2007 on Win XP SP3.  Installation went fine.  I can see the Add-In but it's located in the "Inactive Application" section.  When I click to view available add-ins, it shows but the checkbox is not checked.  When I click on it, I see the following error...  "A runtime error occurred during the loading of the COM add-in."  If i check it, it just resets to unchecked.

Looks like the installation did not go fine. Could you please uninstall it, restart the machine and try installing again?

This problem is happening on 3 different PC's.  I find it hard to believe that the installation did not go fine on all 3 of them.  As I said in my original post, the problem only happens when running Outlook 2007 on Win XP SP3.  All other combinations of OS and Outlook versions work fine.
